This fall, your school and the Weber-Morgan Health Department (WMHD) are partnering to provide their annual Flu Immunization Clinic.
In addition to the Flu vaccine, WMHD will be offering Tdap, Meningococcal and HPV immunizations for 6th grade, junior high and high school students. These vaccines are recommended prior to 7th grade entry and additional doses of Meningococcal and HPV vaccines are needed to complete the full series. Weber-Morgan Health Department nurses will be offering immunizations to all students and school faculty.
Family members are also welcome to attend and receive vaccinations at the same time. We hope this will provide an easy opportunity for students, faculty and families to receive the flu vaccine.
We will not be able to provide a vaccine without a completed consent form. You can complete the consent form online or print a copy. Consent forms will also be available a the clinic.

Once completed, you will receive a confirmation email and the consent form will be securely sent to our staff to print. Forms completed through the survey link will need to be submitted at least 1 school day prior to the clinic.
