Kindergarten Registration and Orientation will be held at 2:00 PM on Wednesday, April 10, 2024.
Pre-Registration
Begin the registration process by completing Kindergarten Pre-Registration Form at:

You may also request a paper copy of the pre-registration form by calling the school office at 801-395-5650.
Required Documents
The following documents are required. Please bring these documents with you to Kindergarten Registration and Orientation on April 10, 2024. You may also bring them to the school office prior to April 10.
To enroll your child in kindergarten, we request the following documents:
- Birth Certificate: Your child's original birth certificate is required for age verification. (we will make a copy for our records)
- Immunization Records: Up-to-date immunization records are mandatory for enrollment.
- Proof of Residence: Please provide 2 current proofs of residence; i.e., utility bills (gas and/or electric), drivers license (if the address is listed on the license), lease agreement or contract as proof of residence.
- Guardianship Documents: If applicable, legal guardianship documents should be submitted.
